Your Health Information: When you receive services from APEX Recovery IV, we create a record of your care. This record includes your health screening responses, clinical assessments, treatment provided, and follow-up notes.

How We May Use Your Health Information: Your health information may be used for treatment (providing and coordinating your care), payment processing, and healthcare operations (quality improvement, staff training on de-identified data).

Your Rights: You have the right to request access to your health records, request corrections to your health information, request restrictions on certain uses of your information, receive confidential communications, and receive an accounting of disclosures.

Our Responsibilities: We are required to maintain the privacy of your health information, provide you with this notice, and follow the terms of this notice currently in effect.

Access Limitations: Your health information is accessible only to the licensed medical providers and clinical staff involved in your care. Gym owners, gym staff, and non-clinical personnel do not have access to individual health records.

Communications: Appointment notifications, reminders, and updates will never contain specific health information. All communications use generic language such as "Your APEX appointment has been updated. Log in to My Corner for details."
